Treating Weight Regain after Weight-Loss SurgeryWhen a person with obesity undergo an obesity surgery, gastric bypass or gastric sleeve surgery, the goal is to reduce the stomach capacity so that the patient has a sensation of satiation and fullness. The procedure meets its goal but it has been proven that, after a few years, problems may arise in some cases.

 

 

One of these problems is that over time, the canal that takes the food from the reduced stomach down to the end of the intestine has dilated. When this happens, patients may not experience that original feeling of satiation , the stomach wall begins to dilate and increase the size allowing a greater amount of food re-entering the stomach. The intervention performed some years back, whose main goal was this, loses effectiveness and the larger the aperture, the more kilos recovered.

Do I need to have another operation ?
When this happens, patients dont´need to have another laparoscopic operation. There is now the possibility of using endoscopic techniques to repair gastric bypass failures.

With specific tools the canal is sutured again and the stomach is reduced once again so that the patient recover the feeling of satiation and fullness.

What are the advantages of the endoscopic procedure?

This procedure is perfomed by inserting an endoscope through the mouth,that means patient benefit from using the endoscopic procedure because is a less invasive intervention than a laparoscopic surgery. It is also fastest (the patient can be dismissed from hospital almost immediately after the intervention and no prior hospitalization is required) and least painful for the patient and it has no associated risks.

Can there be any complications?
It is possible that the canal might contract excessively and this causes some discomfort to patients or prevents them from eating. This problem can be easily solved since, in this case, the specialist dilates the canal slightly so that patients can carry on with their normal lives.

Recently, Dr Jorge Carlos Espinós – a member of our medical team – presented a doctoral thesis on the endoscopic treatment of obesity.

The thesis, titled “The endoscopic treatment of obesity. POSE technique using the incisionless endoscopic operating platform (IOP). Prospective pilot study: clinical efficacy and physiological impact”, is the world’s first publication on the preliminary results of the POSE (Preliminary Obesity Surgery Endolumenal) technique in the treatment of obesity and a study of its physiological mechanisms.

Medical Training in the POSE MethodSeveral studies that have been published (including the first study in the world on the results of the POSE Method), our results regarding weight loss, the effectiveness and maintaining of the weight lost in the long term, and the improvement of diseases associated with obesity, including hypertension and type 2 diabetes, among others, have resulted in our medical team being considered international leaders in treating excess weight and obesity with the POSE Method today.

As such, and parallel to our participation in conferences and the activity of daily consultations, our medical team regularly provides training on an international level in several techniques, but in the POSE Method in particular. Currently, we are the only centre authorised to provide training in the POSE Method, and we have trained different medical teams in Spain, France, the Netherlands, Austria, Saudi Arabia, Kuwait and Dubai, among others.

Under this framework of collaboration and training with other medical professionals, Dr Román Turró recently performed nine POSE Method interventions in Germany and trained four medical professionals in Dortmund (Germany) in this endoscopic technique, which continues to accumulate scientific evidence demonstrating optimal results in the treatment of excess weight and obesity.

Safer Alternative to Weight Loss Surgery Aspire AssistWe’ve already spoken on this blog on various occasions about the Aspire Assist- Non Surgical Weight Loss Procedure- a treatment based mainly on the patient eliminating close to 30% of the foodstuffs he/she has ingested before the calories are absorbed by the body. 

Since we began this treatment in October 2013 – being the first and currently only medical team on a national level to offer it, as well as one of the most pioneering medical teams in Europe – we have followed the development of 14 patients treated with this new technique.

Recently, Dr Román Turró from Hospital Quirón Teknon (Barcelona-Spain) presented the first results based on our experience to other obesity treatment medical professionals at the GOSS Congress hosted by the Emirates Obesity and Metabolic Surgery Interest Group. We also wished to communicate these results through this blog. Furthermore, the results were presented at the recently held SECO (Spanish Society for Obesity and Metabolic Surgery) Congress.

There were 14 patients participating in the study carried out with the Aspire System. Four were men and 10 were women, with all patients aged between 37 and 69 years old, and with an average Body Mass Index of 47.9 (morbidly obese). The weight of each patient varied between 96.5 kilograms and 231 kilograms).

Participants in the study demonstrated different associated comorbidities (hypertension, food anxiety, gout, depression, type 2 diabetes, hypothyroidism, sleep apnoea, etc.).

In all cases, no complications were noted, although slight discomfort was reported in some patients after the planting of the device.

  • All patients attended the planned medical follow-up programme.
  • After three months the average excess weight lost was 25.4%
  • After six months it was 37.4%.
  • After one year the average excess weight lost was 49,31 % 

  • The degree of satisfaction amongst the patients has been very high.
  • All of them, in addition to losing weight, have applied changes to their daily habits.
Further studies are required to evaluate the treatment but, based on our experience, we can say that the results are good, therefore it can be considered a good non-surgical option in the treatment of obesity with very low risk.
